Accessing Plan ID Cards and LiveHealth.

The impact of Hurricane Harvey and Hurricane Irma is significant—making it difficult for Crewmembers to access healthcare in their community. We want to make sure that you can get the care you need quickly and easily. If you’ve lost your health plan ID cards or need to speak to a doctor fast, see below for details.

Medical | UnitedHealthcare and Anthem

UnitedHealthcare

  • For replacement ID cards:
    From your computer:

    1. Login to myuhc.com and sign in or register your account.
    2. Once logged in, click on Account Settings > Request ID Cards > select Covered Member > Print ID Card.

    From your mobile device:

    1. Download the Health4Me mobile app and select “Register.”
    2. Follow the prompts to complete your registration.
    3. Click on “ID Card” to view, fax or email your ID card right from the app.

Anthem

  • For replacement ID cards:
    From your computer:

    1. Go to anthem.com and select the icon above “Already a member? Sign in here.”
    2. Provide the personal information requested.
    3. Set your email preferences.
    4. Follow the prompts to complete your registration.

    From your mobile device:

    1. Download the Anthem Anywhere mobile app and select Register.
    2. Confirm your identity.
    3. Create a username and password.
    4. Set your email preferences.
    5. Follow the prompts to complete your registration.
    6. Share, fax or email your ID card right from the app.

Prescriptions | CVS/Caremark

  • For replacement ID cards:
    1. Log in to caremark.com.
    2. Click Order Prescriptions.
    3. Select Print My Prescription Benefit Card.
    4. Click the Print an ID Card button.
    5. Click Print.

Dental | Delta Dental

  • For replacement ID cards:
    1. Registered users: Visit deltadentalins.com/jetblue and click on Member Login.  Click on Print ID card. Your customized ID card will appear in a new window and ready to print.
    2. New users: Click on Register Today and follow the three registration steps.
    3. Mobile users: Enter deltadentalins.com/jetblue on your smartphone’s browser and click the Visit Mobile Site button to view your electronic ID card.

Vision | Eyemed Vision Care

  • For replacement ID cards:
    1. Visit eyemed.com and click on Member Login.
    2. If you’re a new user, click on Create an Account.
    3. Register using your member ID or the last four digits of your social security number. (You’ll get an email asking to confirm your account.)
    4. Finish setting up your new account with your email address and a password. (To keep it secure, please follow the password “musts.”)
    5. Come back anytime to access your ID card, change your password, email address and billing preferences.

LiveHealth Online | Free visits for Crewmembers and family

LiveHealth Online provides 24/7 online medical access to board-certified doctors AND free 45-minute therapy counseling sessions through licensed therapists at no charge through December 31, 2017. Available to Crewmembers in the aftermath of Hurricane Harvey in Texas/Louisiana or affected by Hurricane Irma in Florida.

  • Use a smartphone, tablet or computer with a webcam.
  • Get quality medical care for common health conditions like a sinus infection, cold, cuts, pink eye, bruises, sprains or strains and other non-emergency issues.
  • Prescriptions and refills can be provided by board-certified doctors as needed.
  • Therapists and psychologists offer flexible daytime, evening and weekend appointments.

If your location is either Texas or Louisiana:

  • Sign up and check your “Location” on the top right corner of the page. To adjust, select Texas or Louisiana from the dropdown and click Save. Select Help for Harvey Medical to visit with a doctor or Help for Harvey Psychology to schedule a 45-minute counseling session at no cost to you.

If your location is Florida:

  • Sign up and check your “Location” on the top right corner of the page. To adjust, select Florida from the dropdown and click Save. Select Help for Irma Medical to visit with a doctor or Help for Irma Psychology to schedule a 45-minute counseling session at no cost to you.
