A Biometric Screening is a test that assesses the state of your overall health. The results of your Biometric Screening can help your doctor identify if you have certain health conditions, such as diabetes or heart disease, or if you’re at risk for developing these conditions in the future.

A Biometric Screening measures your:

  • Weight
  • Body Mass Index (BMI)
  • Blood pressure
  • Cholesterol
  • Triglycerides
  • Blood glucose

By learning these numbers, you’ll know what action you need to take, if any, to improve your health and reduce your risks.

How to earn $150 for a Biometric Screening.

  1. Log in to your Rally account, view your Biometric Screening activity and click Learn More. You will then be directed to Quest Diagnostics where you’ll need to register on the Quest website. If you go directly to the Quest site instead of accessing it through Rally, you’ll need to enter the code jetblue2019.
  2. After you’ve registered on the Quest site, print your Biometric Screening Form. Important: Your form is associated with your account and unique to you with a personalized code.
  3. Bring your form with you to your Annual Physical Exam and ask your doctor to fill it out.
  4. After your exam, submit your form by following the instructions on the front. After your form is submitted, your Healthy Rewards will be deposited into your 2019 Health Account.

Earn a BONUS! If you achieve a BMI of less than or equal to 27, you can earn a $50 LIFT.

If your BMI is 27 or lower, you’ll also earn a $50 LIFT in addition to $150 Healthy Rewards when you submit your form (that’s $200 total!). If your BMI is over 27, don’t worry—you can still earn the $50 LIFT within the year, as long as you lower your BMI to 27 or reduce it by two points through exercising and eating right.
